Can anyone give me a list of producers in Italy that make fior di latte (fresh mozzarella) and provola (smoked mozzarella).

No,
but if you are that serious you may want to consider making the mozzarella yourself. I have made it several times and although there is a trick to it, it can be learned, and best of all, without too much "gear". I have had trouble finding any water buffalo to milk however.....I stick with cow's milk. ha

There are a couple of domestic producers of mozzarella di bufala. Here is one of them, they also have smoked mozzarella. I have not purchased from them so I have no idea of the taste or quality.
